EAP Providers
Interested in becoming a EAP
provider of counseling services for Charles Nechtem Associates, Inc.?
To become a provider, you must be a master’s level clinician or above
and provide us a copy of your license, resume and malpractice insurance.
By joining our network, you will also be simultaneously enrolled in
on managed care network, Oasis Health Care, Inc. as well as
our Prepaid Quality Counseling Services network. A one time
enrollment fee to join our networks of $25 is required. Please
also download our provider application and make your check out to Charles
Nechtem Associates, Inc. The check, application, license, resume and
malpractice insurance should be mailed to:
